Anne....Call your local Better Business Bureau and ask about that registration form. There was just a segment on a local news channel with the BBB about all the scams going on with the vaccine.
My mind works like lightning...one brilliant flash and it's gone.

Beth, I did not register in Maricopa county because I expected to be in New Mexico by Feb 1st, but then NM cancelled my February reservations. (I did register in Deming, where I expected to be.) I have reservations in NM starting Feb 28, but if they cancel March, I have decided I am going to drive to Cochiti Lake, which has a very nice, fairly new, COE campground for a senior price of $10 per night. It is about 25 miles southwest of Santa Fe, so it will be cold there at night, but it is very nice, cheap, and the COE are not closing parks. Plus, the Maricopa regional and state parks are fully booked, and I feel safer out in the desert away from everything so don't want to stay in town or commercial campgrounds.

Right now i am in Dead Horse Ranch SP in Cottonwood and will head back to Willow Beach on Monday for a week. It is about as isolated as possible with sites far apart from one another. If NM cancels March reservations, I will head east across I-40 and stay a few nights at Homolovi SP near Winslow AZ. It also is out in the desert and sites are fairly spread apart. Been there before. And it does not seem to be very full right now. Not quite as cold there as in Cochiti Lake, but the Santa Fe area will start warming up soon anyway.

I am registered at Deming, NM, so if they call me, I will make an appt and get there somehow. Hard to make plans further than that. All I can do is try to stay as safe as possible. This past 12-months have certainly made full-timing challenging because I think I have had at least 60% of my reservations cancelled since I was booted out of a COE campground last March in Florida!!!!!!!!

Just an idea for those of you having trouble getting appointments for the vaccine: I have a friend who doesn't have a computer nor a smart phone and most all the info available was to register on line. Someone told her to contact the local, state or County on Aging, or Dept. on Aging (or whatever it is called in your area) or a local Senior Center that might refer her. She called the county office, waited about 10 min. on hold, but got a nice young man who actually got an appointment for her! She got her first shot yesterday, only 4 days after talking to the guy. For what it's worth . . . . .
